Medical School must have a formal written affiliation with St. Elizabeth Medical Center.
A letter from the dean of the medical school or program director of the participating school must be submitted to the Family Medicine Residency Program Director at St. Elizabeth Family Medicine Residency Program prior to the start of the rotation. The letter must indicate approval of the rotation and a list of approved activities including level of supervision
Immunization and health records must be submitted to St. Elizabeth Medical Center Employee Health Office prior to start of the rotation.
Proof of a minimum of $1,000,000/3,000,000 in medical liability insurance coverage must be presented prior to start of the rotation.
All rotation electives and/or required rotations must be scheduled through the Residency Office.
All medical students must complete SEMC mandatory in-service training requirements prior to beginning their rotation. They must wear a picture ID badge at all times while on the SEMC premises.
